SECTION 209-u OF THE GENERAL MUNICIPAL LAW
This is in reference to our Circular Letter No. 16(1980), in which we advised insurers of the availability of the required forms, including instructions, from:

Such office is presently experiencing distribution problems in supplying these forms.
We are forwarding herewith a copy of a letter which Mr. Harris, Director of Program Planning and Administration, Office of Fire Prevention and Control, has requested that we send to all insurance companies authorized to write fire insurance in New York State.
Your adherence to Mr. Harris" request, including the designation of a single person and/or supply office responsible for your company, will substantially reduce the form distribution problems.
