PROCEDURES FOR THE FILING OF POLICY FORMS, RULES AND RATES: OPTIONAL PROCEDURE FOR OBTAINING ACKNOWLEDGEMENTS OF FILINGS

Circular Letter No. 11, issued May 7, 1998, provided information and guidance on the procedures to be followed by insurers filing policy forms, rules and rates, in order to ensure that the required review and approval or acknowledgement of such filings is completed in an expeditious manner. The purpose of this Supplement is to advise insurers of the optional procedure to follow if they wish to receive an initial receipt for a filing.

Insurers should complete the Insurer Information Section on the attached form (PDF file) in duplicate and submit it with the filing, along with a self-addressed postage-paid return envelope. The Department will stamp and date the form, and will provide the file number that has been assigned to the filing. If the filing has already been assigned to an examiner, the examiner’s name and telephone number will also be provided on the return copy.

Please note that the returned copy of the form will only serve as evidence that a filing has been physically received by the Department and that the review process has commenced. In no event should it be construed as an indication of final disposition or approval of the filing.
