RE: PRINTED ANNUAL AND QUARTERLY STATEMENT FORMS TO BE FURNISHED BY THIS DEPARTMENT

Pursuant to Sections 307 and 308 of the New York Insurance Law you are required to file Annual and, where required, Quarterly Statements with this Department. These sections provide that the Department is to have prepared and furnished to you, all such forms and schedules which differ from or are in addition to those adopted from time to time by the National Association of Insurance Commissioners.

In the past, all forms and schedules required to be filed have been printed and furnished by the New York Insurance Department. Please be advised, however, that beginning with the Annual Statement as of December 31, 1988 and thereafter, only those forms and schedules which differ from or are in addition to those adopted from time to time by the National Association of Insurance Commissioners shall be printed and furnished by this Department.

Three (3) copies of each form or schedule shall be furnished by this Department; it shall be your responsibility to obtain all other forms and schedules required to be filed.
