SUBJECT: REPORTING REQUIREMENTS - PRODUCT LIABILITY INSURANCE

Circular Letter No. 26 (1979), dated September 26, 1979, included forms for the reporting of claims, cancellations and nonrenewals which were to be completed for the six month period ending December 31, 1979.

The same forms are to be completed for the six month periods ending June 30, 1980 and December 31, 1980. The filing date for transmitting these reports to this Department shall be no later than 60 days following the end of the respective six month periods.

The completed forms, signed by a responsible officer of the insurer, should be mailed to:
