Submission of Biographical Affidavits and Fingerprints for Newly Elected Directors and Officers of Licensed Property and Casualty Insurance Companies
The following procedures apply to licensed property and casualty insurance companies. Contact Capital Markets Bureau regarding the procedures for financial guaranty and mortgage guaranty insurance companies.
Submission of biographical affidavits and receipts for electronic fingerprints completed through a New York State Authorized IdentoGO location
Companies should submit a cover letter, the NAIC biographical affidavits and IdentoGo receipts electronically through the Department’s Insurance Company and Fraternal Benefit Society Filings Portal.
The “Applicant Company Name” entered on the biographical affidavits must be a New York licensed insurance company.
The biographical affidavits must have wet signatures and a scanned copy must be submitted to the Department. Electronic signatures and electronic notarizations are not acceptable.
Submission of original fingerprint cards, original IdentoGO pre-enrollment forms and biographical affidavits:
The cover letter and completed biographical affidavits should be submitted electronically through the Department’s Insurance Company and Fraternal Benefit Society Filings Portal.
The “Applicant Company Name” entered on the biographical affidavits must be a New York licensed insurance company.

The IdentoGO pre-enrollment forms should only be printed and signed after the biographical affidavits have been submitted to the Department.
The cover letter along with the original fingerprint cards and original IdentoGO pre-enrollment forms should be mailed to:

DFS Portal
To access the DFS Portal, you must have a portal account. Select the button below to log into or create your portal account. (You may need to disable "pop-up" prevention applications)
Once your Portal account has been created, you will receive an email with a temporary password to log in. When you first log in, click on Ask for Apps to request access to the Insurance Company and Fraternal Benefit Society Filings application, for the company(ies) you will be filing for. When this is approved, you will be able to access Insurance Company and Fraternal Benefit Society Filings application in the My Apps menu.
