Life Company Accreditation Checklist
One set of the following original documents should be submitted to the DFS Office of General Counsel in Albany and one set to the Department's Life Bureau in New York.
- Certified copy of the Company’s charter (articles), certified by the state of domicile.
- Certified copy of the Company’s by-laws, certified by the corporate secretary or by the state of domicile.
- Certified copy of the Board resolution pursuant to Section 1106 of the New York Insurance Law. (Section 1106 resolution)
- Certified copy of the Company’s latest report on examination.
- Annual Statements for each of the three years immediately preceding the year of application.
- Schedule S, Part 4 prepared on a New York basis for 3 years.
- Interim financial statements.
- Certificate of Compliance from the state of domicile.
- Certificate of Deposit from the state of domicile.
- * Copies of all reinsurance agreements.
- * Copies of management agreements, if any
- A list of the Company’s stockholders showing the number of shares held by each.
- * A list of the Company’s directors and key officers together with biographical questionnaires on each using UCAA form #11 NAIC Biographical Affidavit.
- Untrustworthy commitments from company and its ultimate parent stating that any director or officer found untrustworthy by the Superintendent will be removed. Untrustworthy Commitment Applicant | Untrustworthy Commitment Parent
- Valuation Certificates.
- Holding Company registration statement.
- Explanation of the Company’s reasons for seeking accreditation at this time and the major sources of its business.
- A statement that the Company currently is in compliance with and will continue to comply with New York reserve requirements including, but not limited to, Regulations 56, 126, 147 and Section 4217 of the New York Insurance Law.
- Holocaust filing under Section 2705 of the New York Insurance Law, if applicable.
- Derivative Use Plan filed under Section 1410 of the New York Insurance Law, if applicable.
- RBC reports for the last three years.
