Per Circular Letter 15 (1999), Annual Statement instructions and blank forms will not be mailed to licensed companies by New York State. A complete set of general and detailed specific instructions, together with blank forms and other related supplemental/addendum forms for the New York Supplement and New York Statements, will be posted on the Department's website in January 2014. Download and print capabilities will be available.
